Northside Community Service - Aged Care Housing

Northside’s Assistance with Care and Housing program is funded throguh the Commonwealth Home Support Program and provides a wide range of services to older community members who are homeless or at risk of being homeless as they get older. This might be because they are on a low income or have insecure housing and face challenges finding a place to live that they can call home. The program team supports individuals over 65 years and people aged 50 years or older who are are prematurely aged*, on a low income and are homeless or at risk of being homeless. Northside Community Service - Common Ground

This unique housing initiative provides supportive and sustainable housing to low income earners and those who have experienced homelessness. As tenancy and project managers for the recently completed Common Ground Canberra, Located in the heart of Gungahlin, close to services, transport and shops, the site comprises of 40 x 1-bedroom units. There are also expansive internal and external common areas available for all tenants to use whenever they like – such as the communal open plan kitchen, computer work stations, entertainment room, art room, large communal veggie gardens and BBQ area. Along with Argyle, Northside Community Services are available onsite to provide that extra support to tenants.

Northside Community Service - Housing First

Housing First recognises that stability is more likely to be achieved if an individual or family has and affordable, safe, permanent place to live. This approach harnesses the power of permanent, safe housing to help people recover and heal from the trauma of homelessness. It gives them a safe space in which to address the issues that may be stopping them from living the lives they want.

Northside Community Service - Majura Men's Shed

Majura Men's Shed build and repair wooden toys, made to order toys and furniture for playgroups and preschools and also for Aged Care & Community Care Facilities. Majura Men's Shed works closely with Alzheimer's Australia ACT to offer a number of programs for men with early onset dimentia.

Northside Community Service - Y.Engage

The Y.Engage team at Northside Community Service are committed to supporting young people of Inner North and Gunghalin regions of Canberra, between the ages of 12 - 25 who are vulnerable and disengaged. The Y.Engage team support young people through outreach, short-term case management, and through providing them with opportunities for leadership and skill building.

Northside Connect

Northside Connect aims to build and encourage the local aspirations our north Brisbane community. Based in Nundah, we provide services such as legal advice, domestic violence support, practical relief from life stressors as well as a safe place to connect and reinforce the sense of community.

Northway Community Playgroup

Volunteer run community playgroup serving a need for families with pre kindy children to learn to socialise as they develop. We have mums with newborns coming who benefit from new friendships made. We do not charge any fees just a voluntary gold coin donation. Playgroup has been operating for 15 years and our outdoor equipment is old and tired. The group is part of the Mission Outreach of Northway Uniting Church, Beldon WA 6027. We provide a service for families restricted by financial situations and fill a gap where they are unable to attend day-care or prefer not to send them. It provides opportunities for volunteers to participate in community activities and enhance their self-worth and enjoyment of life.

Northwest Community Group Inc

We support people experiencing or at risk of experiencing homelessness. We do this through 3 x stands per week where we give out clothing, toiletries, blankets, tents, sleeping bags, air matresses etc.

We also run a program to help people who have gone from homelessness to housing, to help them get items they need to make their house a home.

Nourishing Neighbours

Provides culturally appropriate emergency food relief to those experiencing food insecurity. All clients are via support service referral ensuring their need is genuine & they have a team supporting them.

NOVA for Women and Children

NOVA for Women and Children is a specialist homelessness and domestic violence service for women and children, in the Newcastle and Lake Macquarie areas. A service they have been providing for over 35 years.

Nova has 4 refuges spread throughout the region, providing crisis accommodation for up to 80 women and children each night who may be homeless or escaping domestic violence. Along with this they provide clients with a specialist case manager, an outreach a referral service and assistance in numerous ways to help women and families get back on their feet and start over.

Novo Education Space

A non-government Special Assistance secondary school in regional NSW. Our school strives to lift up young people, developing in them renewed hope, self-worth, autonomy and optimism. Our job is to help young people believe in themselves and to realise that they can shape their future and choose what it will be, regardless of what has come before. Nowra Community Food Store

NCFS is a not for profit charity, supporting residents of the Shoalhaven through providing low cost and free groceries to those that may be experiencing economic hardship. We also provide free grocery and personal care hampers to those in need. We have supported the community through the recent natural disasters in our area, now through the Covid19 pandemic.

NSW Health - Aboriginal Child and Family Health

Within Aboriginal Child and Family health there are teams of Child and Family Health Nurses, Aboriginal Health Workers, and Social Workers in Community providing telehealth and sustained home visiting programs. We work along side families who may be experiencing child protection, financial, homelessness, mental health or social concerns. We have a culturally safe, trauma informed and holistic approach to enhance family functioning, child development and community wellbeing.

Bulbuwil - chronic diease prevention programs for Aboriginal and Torres Strait Islander people in the St George and Sutherland Shire areas of Sydney. Offering numerous healthy lifestyle programs, exercise groups, cooking groups, art therapy groups, eye and hearing clinics. Health checks and referrals for follow ups.

SEAHC - care coordination for Aboriginal and Torres Strait Islander people in the South Eastern Sydney Local Health District with chroinc dieases. Cooridnating specialist, doctors, applications to NDIS and my aged care, advocating for patients.

NSW Women in Dairy

The NSW Women in Dairy aims to provide a state-wide structure by which to enable and uplift women in the dairy industry, connecting them to support for mental health, social wellbeing, learning opportunities and professional and personal growth and leadership. At its core, the organisation strives to support and enhance women’s contribution to the dairy industry and empower women of all ages to positively and proactively offer change in the industry. We collaborate statewide with a wide range of organisations including agricultural industry, government and other non profit providers to design, develop and deliver initiatives that are grounded in foundational principles of mentoring, personal development, leadership and social cohesion. The organisation is led by a seven member committee, supported by an extended volunteer network from across the state. Our major annual even is the women in dairy Bale Up Conference.

Nungurner Primary School

Nungurner Primary School is a small school in East Gippsland with 33 students. Although our school was not physically impacted by the 2019/2020 bushfire season our local community has been deeply impacted and financial losses to our community have been significant. Our students experienced multiple evacuations from their homes over the summer and this has impacted on our students' and families' wellbeing. The flow on effect to local businesses and the impact of COVID-19 has meant that our school has been unable to reach out to local community members and businesses to raise much needed fundraising for small projects within our school. We are seeking external support through GIVIT to assist us to undertake some small projects within our school, including the rejuvenation of our vegetable gardens and playground.

Nurse NavigatorsIn 2016 the Nurse Navigator program was introduced in Queensland. Led by the Office of the Chief Nursing and Midwifery Officer, Clinical Excellence Division, the Nurse Navigator Service provides dedicated roles to facilitate the journey of patients with complex healthcare needs through an increasingly complex health-care system.

With the GP as the primary care provider and case manager, the nurse-led model of care provided by Nurse Navigator Service assists patients to navigate the complex healthcare system, from the community or primary health care setting, through hospital, and back home again. By working across multiple specialties, the Nurse Navigator Service aims to reduce service fragmentation and improve access for patients through care coordination, advocacy and education.

As highly experienced clinicians with an in-depth knowledge of the health system, Nurse Navigators support patients to improve their health literacy and capacity to self-manage their conditions. Nurse Navigators assist patients to access services in the right place, at the right time and by the right health-care professional or care provider.

Nurse Navigators link with all primary and acute care health professionals who identify and refer their most complex patients (adult or children), who require a coordinated approach to improve their health outcomes. Based upon similar criteria, other referrals to the service will come from specialists, hospitals and hospital data bases.

We are a small country church active in serving the local public and high school through chaplaincy and in youth and children's ministry. We also manage a community hall used by clubs and individuals for social gatherings.

Our Mission is to provide street level early social intervention to Aboriginal people who are at risk of self harm, low level violent behaviour, entering the criminal justice system, homelessness, substance abuse or mental health issues and, through leadership and partnership with other organisations, to ensure that issues and unmet needs are met.

On My Feet

On My Feet is dedicated to helping people experiencing, or vulnerable to homelessness, transform their lives by providing a unique hand-up comprising exercise, community & education. We collaborate with shelters, who help us “recruit” participants. Participants can choose to join morning walks with free coffee/chats afterward, and structured training sessions on Tuesday and Thursday evenings. They receive our T-shirt & socks, & gain access to a network of caring, friendly volunteers. Participants experience improvements in their physical and mental health & build strong connections with the community & one another. These connections are significant determinants of well-being. Participants are entered into running/walking events to build their confidence We offer mentorship, as well as learning opportunities i.e. a regular schedule of workshops on topics such as resilience, nutrition, stress management & sleep. We work closely with employment agencies to help participants find work.
